Daily Living & Independence Skills

- Household management and cleaning

- Use of public transportation and community navigation
Community Integration & Participation
- Accessing community resources and activities
- Building and maintaining social relationships
- Participating in recreational and leisure activities
- Developing natural supports and community connections
- Engaging with civic and cultural community life
Transitional Support
- Transitioning from school to adult services
- Transitioning from institutional or residential settings to community living
- Transitioning between living arrangements
- Building skills needed for greater independence in a new environment
- Coordinating with schools, residential providers, and other agencies
Communication & Self-Advocacy
- Developing effective communication strategies
- Building self-advocacy skills
- Learning to navigate social situations and relationships
- Using assistive technology for communication when applicable
Eligibility
Guided Path Support typically serves adults with intellectual and developmental disabilities (I/DD) who are seeking to increase their independence and community involvement. To receive ITS services under the 1915(i) HCBS SPA, individuals must meet:
- • Active NC Medicaid enrollment
• Documented I/DD and/or qualifying behavioral health diagnosis
• Meet NC Medicaid 1915(i) level of need criteria
• Enrollment with Alliance Health BH I/DD Tailored Plan
• Valid service authorization from Alliance Health
• Active Person-Centered Plan identifying ITS goals
• Signed consent from client or legal guardian
• Residing within Mecklenburg County, NC
